How to Add/Update Assets

Asset Coordinators (within the M-NCPPC Department of Parks and Recreation Prince George's County) begin the process of adding new assets new by filling out the Capital Asset Input Form.

Users/Asset Coordinators must legibly fill out this form to the best of their ability (in pencil or online PDF) in order for any request to be filled.

This form should only be filled for 3 types of asset transactions. Users/Asset Coordinators must check the transaction type from the highlighted sections on the form below then fill out the corresponding section.

  1. New = For All Capital Assets
  2. Disposal = For Disposals
  3. Transfer = For Transfers

Instructions: To report a new addition to our Capital Assets, complete Sections I and IV. To report a disposal of our Capital Assets, complete Sections I, II, and IV. To report transfers of Capital Assets, complete Sections I, III, and IV. When completed, send this form to the Department of Finance, Capital Asset Accountant.

The following sections are mandatory within Section I:

+Property Tag # (shown on the red label attached to your asset)

+Location Code (Procurement Code) and/or Location Name (Nickname)

The following sections are mandatory within Section III:

+Transfer FROM (Location Code and Name)

+Transfer TO (Location Code and Name)

4. Authorization

Section IV of the form showing authorization signature lines requiring two signatures.

Section IV requires at least 2 signatures, one being a supervisor.
